No database notes on the promo single, but there definitely were different mixes of "Complicated." My CHR at the time always played the "Tom Lord-Alge Mix," although when I took a job at my next station, we always played the album version (aka "The Matrix Mix"). My copy of the "Tom Lord-Alge Mix" on a Promo Only CD runs 4:03.

EDIT: The "Tom Lord-Alge Mix" is actually the album version.

Interestingly, I found info online that indicates both a US 7" and US CD single was issued with the "Tom Lord-Alge Mix" as the A-side / lead CD track.

Aaron, I only currently own one promo CD single for this song, and here are my promo's details:

-Avril Lavigne-"Complicated" (ARPCD-5099)
1-The Matrix Mix (listed 4:03; actual 4:02)
2-Tom Lord-Alge Mix (listed 4:08; actual 4:05)
3-(callout hook)

Although I don't currently own it, my friend Tom has a second promo CD single listed (ARPCD-5116). His description of this CD is (4:06 + 3), which means one, 4:06 version of "Complicated", followed by 3 other songs by her - this could be the one you saw online, sir.

My copy of the Let Go CD (which is a Canadian pressing, but I doubt it would differ from the U.S. pressing since Avril Lavigne was signed directly to Arista US), states that "Complicated" was produced by The Matrix but mixed by Tom Lord-Alge. This version is identical to the "Tom Lord-Alge Mix" on the Promo Only CD.

There was indeed a commercial CD single... I bought it when
it came out.

Paul, you are absolutely correct. My post from several years ago had the album version matched to the wrong mix. To correct myself above, WDRQ where I worked in Detroit played the "Tom Lord-Alge Mix," which is the album version. When I worked for Radio Disney, we always played "The Matrix Mix," which has a different drum track and some other noticeable mix differences.

I'm actually quite surprised that Arista put "The Matrix Mix" as the lead track on the promo CD. In my opinion, the album version (aka "Tom Lord-Alge Mix") is much more polished and sounds like a radio mix.
