I have two different promo CD singles for "As I Lay Me Down" by Sophie B. Hawkins. Here are the details:
CSK 77801
1. non-described version (listed & actual 4:09)
CSK 7280
1. Airplay Mix (listed & actual 3:26)
2. Rhythm Mix (listed 3:49; actual 3:48)
3. "Butcher" Mix (listed & actual 3:49)
I'm betting many stations played the non-described version from the first promo CD, which was also the only version issued by TM Century. Our station, however, only ever played the Airplay Mix from the second CD.

Is the Airplay Mix just an edit?

Nope, different mix. Instruments have been shifted around in the mix, and the vocals are completely dry on the Airplay Mix.

ISTR the 2nd CD was serviced somewhere mid chart run. We
switched to the rhythm mix around that time.

It's possible my station was late on adding the song, because I don't
ever recall playing the 4:09 album version.

For what it's worth, when Top Hits USA put the song on their recurrents series (disc number RH031), they used the 3:26 Airplay Mix.

I also have it on Top Hits. And, as usual for Top Hits,
my copy is faded early and only runs 3:21.

Today I heard 106.1 WISX in Philly play a version that has extra instrumentation and more prominent "ooh la kah koh" background vocals. Is that the "Airplay Mix"? It's the first time I've ever heard anything other than the album version.

Wow, I don't recall anything but the album mix... I
believe that's what the music video used, as well.
EDIT: Ah, I see that there were at least 2 music videos.
One that used the album version and another that got
rotation on MTV and used the 3:25 edit with dry vocals.

I always heard the "Album Mix" in the 90s/early 2000s in Philly on Star/Alice
1045. I'm guessing the "Butcher" mix was an attempt to get it Alternative
airplay, though it's really a pop song.

Yeah, if anything, maybe Triple A / Adult Alternative might have played it. Joe "The Butcher" Nicolo (mainly known for producing hip-hop) did all the remixes on the 3-track promo, and the "Butcher" Mix sounds the least pop. It's a stripped down mix that removes the synthesizers and focuses heavily on the percussion.

Is there any consensus on whether the airplay mix or album version got the most play on the radio?

At the time the song came out, I was a subscriber to Top
Hits U.S.A. They only serviced the Airplay Mix.
