I compared my 45 version to an mp3 of the album version (runs about 5:02 as best I can tell), and the bottom line is you can't recreate the 45 from the LP version.
You can recreate the two edits exactly, but if you do, your mixdown will fade too soon - the 45 has more vocal ad-libbing at the end than the LP version does.
To recreate the two edits:
Keep 0:00.0-2:29.0 of the LP version.
Edit on snare at 2:29, 3 beats before the downbeat.
Remove 32 beats from 2:29.0-2:44.7.
Keep 2:44.7-4:09.4 of the LP version.
Edit on snare at 4:09, 3 beats before the downbeat.
Remove 64 beats from 4:09.4-4:40.9.
Keep 4:40.9-5:05 (end) of LP version.
Watch in horror as edited LP version fades about 15 seconds before the 45. Oh well.

Yes, Ed, I do have the promo CD single (CSK 73047), with no listed times included. Here are the details:
1-(Single Edit) (actual 4:28)
2-(Album Version) (actual 5:02)
