My commercial 45, confirmed as Dial 4063, has a listed time of (3:10) and an actual time of (3:11). I only post this info because current database CDs for this song feature timings ranging from (3:02) to (3:12).

my commercial 45 has 0:02 of unique audience chatter
before
the song starts that is different than what is on the
"very
best of" cd....the 45's first drum beat is at 0:04 of the
song while the cd versions first drum beat is at
0:02....pat, do any of the cd's have the proper vinyl 45
version?...it looks like the cd "his best" may have the
proper version because of it's longer (3:11) running time
versus the others (3:03-3:04)...i just ordered the "his
best" cd from amazon and i will let the board know of my
findings when it arrives...

The version on "His Best" is identical to the 45. The first drum beat is at :04 and the total time is (3:11). Rhino's "Very Best Of" has a slightly shorter audience noise introduction such that the first drum beat is at :025 and it also fades the song sooner than the 45 for a total running time of (3:04).
There is no audience noise preceding the start of this song on the LP version.

Does that include during the instrumental intro (in other words, could the first couple of seconds be lopped off to create the LP version intro, or is
the whole thing completely different)?

The audience noise overlaps the opening drum beats, and would still be present if you just edited off the first few seconds of the song. (I don't have the LP version to compare, though.)
Mastering notes:
Mono 45 version
I have the mono version on just one CD - Atlantic Rhythm And Blues 1947-1974 Vol. 6 (1985 release, not the 1991 rerelease). It sounds fine here (nice EQ, nice dynamic range, no noise reduction artifacts on the fade), and runs 3:09. The intro is shortened a bit, so that the opening drum beat is at 0:02.
Stereo 45 version
The oldest mastering I have of the stereo 45 version is on Rhino's Best Of (1988). It also sounds quite nice here (nice EQ, nice dynamic range, no noise reduction artifacts on the fade), and runs 3:05. The intro is also shortened so that the opening drum beat is at 0:02. Here, the fade starts much earlier and ends much earlier than the actual 45. The same analog transfer is used on:- Rhino's Billboard Top R&B Hits 1967 (1989) - digitally identical
- Time-Life's Classic Rock Vol. 24 1967 Blowin' Your Mind (1990) - left/right channels reversed
- Rhino's cheapie Soul Hits Vol. 2 (1993)
- Varese Sarabande's Rock And Roll The First 50 Years The Late '60s (2005) - digitally exactly 1.2 dB quieter
I do have one disc with a different analog transfer - Atlantic Rhythm And Blues 1947-1974 Disc 7 (1991 rerelease, not the 1985 original release). Sound quality is also quite nice, like all of the above. The opening drum beat is at 0:02. It runs 3:09, which leads me to think that the fade points match the actual 45.
