I have searched high and low, and to my knowledge, the exact US 45 edit has never been issued on CD...

But the good news is that it's easy to recreate with a simple audio program, such as Magix Audio Cleaning Lab I mentioned in the "Amadeus" thread. All you have to do is shave off the front of the 3:30 version up to where the single starts, and you've got the 45 version.
