I can't comment on the remix issue, but I can answer this.
The 45 version was indeed included with at least the
initial pressings of the vinyl LP. True, it's not on the
actual LP itself. Much like "Rock and Roll Dreams Come
Through" on Jim Steinman's "Bad For Good" LP, the 45 was
included as a separate 7" insert.

I still have my copy of this insert from when I bought the
LP at the time it came out. It lists a run time of 3:13
(which I have never timed for confirmation) and the note
"Special Complimentary Single; Not For Resale" across the
top of the label. Another unique thing about it is that is
a truly one-sided single. No flip side. The back of the
single is just smooth vinyl with a blank label. Also
unusual for a 7" single, it plays at 33 1/3 instead of the
usual 45 RPM.

So, the single was included with the LP. Recently I was working on this song and going in I had
the assumption that the Cd appearances running (3:05) to
(3:09) were simply the DJ 45 version. The actual DJ 45
(3:06)is in fact just an early fade of the stock 45,
which runs (3:13).
There is no "LP Version" since this song was never
included in the soundtrack LP (despite the the picture
sleeve clearly stating it was). It was included in some
cassette pressings from 1978, and then as a bonus track
on the Varese re-issue of the soundtrack.

But when I compared the 45 closely with the CD, I
discovered that the version on CD is neither the 45 nor
promo 45 at all. It is a previously un-heard alternate
mix. With Jim and Pat's help I found that all 3 of the
Cd's in the DB contain this remix. I must note that the
mix differences are subtle - I had NO clue this wasn't
the single until I really put this under a microscope.

The main differences are as follows:
1) In the intro of the 45, a keyboard plays the 5 theme
notes twice, then the music starts on a down beat.
But on CD, the opening notes play twice and we hear a
tom-drum hit before the down beat. The drums also start
about 1/2 second sooner on the Cd as well.
2)On the 45, at about :28 and again at :33, there are 2
loud concert-bass drum hits. But in the same places on
the CD mix, those drum hits are missing.
3)The CD master has a generous amount of reverb added to
the left channel - the 45 does not have this reverb.

I assume when this was first digitized for CD, Arista
pulled a wrong tape. Maybe that mix was issued in another
country - who knows?
So bottom line is, the only place to get the 45 version
is from a 45 dub.

Did we ever get this figured out, why exactly the commercial single version runs four to six seconds longer than it does on any CD? Does the commercial 45 run faster, or is it an early fade? This is a good question, David, given that DJ copies of "Theme from 'Close Encounters of the Third Kind'" are reported to run 3:06 and all of the song's CD appearances in the database run from 3:05-3:09.

There's no notation for the versions listed in the database.
