It's possible to fix the fade in to a non-fade in. As long as the entire song is intact, and there's nothing missing from the beginning, it can be done. If you crank up the volume at the beginning, can you hear the first note of the song?

Of course, the down side to cranking up the volume during the fade-in is you could wind up with an overabundance of tape hiss in the process.

It depends on if the song was faded in when it was being mastered to an analog tape or not. If it was faded in on a digital master, then you really wouldn't have much excess tape hiss. Furthermore, I've had good results using this process and then removing some of the extra tape hiss using Cool Edit Pro. Just for fun, I created a non-fade out version of "Glory Days" by Springsteen. I was able to remove the ending hiss, making it sound really nice!

While on the subject of "You Could Have Been A Lady" I notice that I have "LP version" listed by the only appearance of this song on cd in the U.S. Don't know where I came to that conclusion since I do not have the vinyl LP that this song originally appeared on. What is the title of that vinyl LP so I can obtain a copy? Anyone know of 45 vs. LP differences for this song?

The parent album is titled "April Wine" and was issued in 1972 on Big Tree 2012 (it "Bubbled Under" at #205). I'm not sure what the difference (if any) there is between album vs single version.

I know this doesn't answer anybody's questions about April Wine's "Could Have Been a Lady", but while we're on the subject... On my copy of Dick Bartley Presents Collector's Essentials: The '70s CD (Varese Sarabande 5706) the song's ending is severely truncated. (I had to smooth out the fade on my Adobe Audition software.) Just want to check and see if the ending is truncated on anybody else's CD copy? A record label with a good reputation like Varese is normally more careful about this kind of thing.
