I found a third label variation that I don't have, but it
appears to be a Monarch pressing:

None of these 45s state "re-service" or anything that
would indicate a different version. I would certainly
have expected to see "Short Version" or something if a
pressing had different audio. Otherwise, why go to the
trouble?

I checked at Discogs and this is pic is the one closest
to my promo as I remember.

it was a vinyl while it seems the Monarch pressing looks

That's one of the copies I have, and it's pressed on vinyl (not polystyrene).
