Stoned Soul Picnic promo 45 PS?

   In the dim recesses of my mind I seem to recall a white paper PS for the promo 45 "Stoned Soul Picnic" on which were instructions on how to do the surrey/surry" dance. 

It was not the same as the commercial PS.

I may even have it but...if so, it's boxed up.

Can anyone shed some light on this?
