Is the 45 version an early fade of the LP version or is there some edit points on
it?

Pat's database indicates "LP version" and "45 version," so most likely it's an edit and not an early fade. As soon as I get a chance, I'll compare the two and let you know where the edit points are.

The 45 has the quiet LP intro chopped off,
and the 45 may be pitched up a bit, too.

Andy is correct. Remove the first 24 seconds of the LP
version. With what you have, it will be 4:06, so speed it
up to run 3:59.
