I timed an 18 second intro on my copy.
I fade it out early at the end of song comes out at
3:54.
I wonder if that is the only difference.

According to Discogs, most of the 7" records have a
duration of 3:54.

I'm pretty sure 3:54 is an incorrect run time. There's a video on YouTube of the 45 being played, and the music seems to start at :07 and fade
out at 4:22, which would total about 4 minutes and 15 seconds.

Hearts 45 RPM

That's not to say that there couldn't be any stock or promo copies out there that actually do run 3:54, but my guess is that they're all in the
4:15-4:18 area for actual length.

thank you for the youtube link, Dan.   Unless someone
has the actual promo 45 in hand and has timed it to
3:54, I would agree this falls in the category of a
mistimed label.

FWIW, my Canadian 45 has a 3:54 printed time, but my
audacity file of it runs 4:15.
